Application Requirements

Applicants must be at least 18 years old.
Applications must include a video submission and a completed application form.

IMPORTANT
Registration for the Pole Instructor Certification – Intermediate Level is now open.
This program is exclusively available to those who have completed and passed the Basic Level certification.

Video Submission

Before starting, make sure that your video submission is ready.

Provide your video submission link through the form below or file by email at [email protected]. If you provide a link, ensure that the video settings are set to public so that we can access the video.

Video Requirements
Submitted video should include the following combos:

Combo 1: Spin Climb - Aerial Invert - Outside Leg Hang - Butterfly - Jade
Combo 2: Front Knee Hook - Climb - Jamila - Flatline - Superman

Demonstrate each skill with clean lines and correct technique.

Videos should feature only the applicant; no other person should be visible.

I confirm that I am at least 18 years old.

I confirm that I submitted the video submission by email or included the link below.

I confirm that all information provided is correct. I am in good physical condition and have the physical capacity to participate in all fitness activities in connection to the program. I understand that pole and aerial are physically demanding activities and I will inform my instructors immediately of any health concerns or issues I have. I understand that some parts of the course may be physically strenuous and I voluntarily participate with full awareness of potential injury risks. I release Milan Pole Dance Studio and its subsidiaries from any and all liability should I injure myself during this program. I understand the program fee is non-refundable and that successful completion does not guarantee a position as an MPDS instructor. I understand that my spot is confirmed only upon review. If I am selected, a 50% non-refundable deposit, payable by credit card, is required to secure my spot. The remaining balance is non-refundable and will be automatically charged on January 3rd, 2026, to your credit card on file.

If I do not successfully complete the program, I understand that a reevaluation is possible the following month at an additional cost of $125.
