Demonstrate each skill with clean lines and correct technique.

I confirm that all the information provided is accurate. I am in good physical condition and physically able to participate in the activities included in the program. I understand that pole and aerial activities are physically demanding, and I will immediately inform my instructors of any health concerns or conditions. I understand that certain parts of the program may be physically challenging, and I participate voluntarily and with full awareness of the potential risk of injury. I release Milan Pole Dance Studio and its affiliates from any liability in the event of injury during the program. I understand that all program fees are non-refundable and that successful completion of the program does not guarantee a position as an MPDS instructor. I understand that my spot is confirmed only after my application has been reviewed and I have been selected. Once selected, a non-refundable 50% deposit, payable by credit card, is required to secure my spot. The remaining balance is non-refundable and will be automatically charged to the credit card on file on February 27, 2027.

If I do not successfully complete the program, I understand that I may be reassessed the following month for an additional fee of $125 or 90 EUR
